A leading nationwide provider of disability case management is hiring a Bilingual Disability Case Manager to work from home. in Ontario and Quebec.

The Disability Case Manager will be managing a variety of complex disability claims (STD, LTD, WSIB) for the purpose of facilitating and expediting appropriate care and recovery to health and return to work if appropriate.

The Disability Case Manager will be responsible for assisting employees on behalf of the employer when the employee is off work or at risk of being off work for health reasons. The Case Manager will assess any barriers to a successful and timely return-to-work and engage with stakeholders to implement strategies to address those identified barriers and support return to work or an appropriate alternative plan.

Duties: 

Conduct case assessments, contacting the employee and their direct manager by telephone to explain the disability management process and gather information for assessment purposes

Develop and document and understanding of the situation and the factors supporting as well as inhibiting a successful return to work for the employee

Provide any of the following interventions: more in-depth assessment, medical management interventions, initiation and coordination of occupational and/or psychological interventions, utilization of specialized resources or consultations, etc.

Determine and appropriate RTW goal with the employee and the employer

Provide active support for the return-to-work planning process between the employee and their supervisor and intervene when necessary

Communicate proactively with the employer’s HR representative regarding and case management issues that may impact the workplace and RTW planning

Management the transition to LTD and provide medical and summary case management information to the LTD insurer to ensure a seamless transaction

Attend Case Management and Disability Management department meetings

Review of information to determine appropriateness of diagnosis and treatment in conjunction with medical guidelines

Assessment of client via telephone

Facilitate appropriate medical assessments and treatments

Review estimated duration of disability and determine appropriateness based on medical diagnosis, treatment and determine readiness for return to work and assist with return to work planning and management as required.

Requirements:

100 % Fluent in French and English

3 years comprehensive experience in Disability Case Management or combination of experience and specialization education.

Registered professional status with the disability management or professional health sector along with course work in occupational health, rehabilitation legislation related to disability case management, disability insurance.


Salary Range: $65,000 -$70,000
Three week’s vacation plus benefits
